Joint pain  can arise from a wide  variety of causes. Depending on  the problem, one or more joints can become inflamed  or painful.

The joints in our  body are complex structures consisting of  bones, muscles, ligaments, tendons and other tissues. Injury or damage to a joint can involve any of  these structures and because of this, it is sometimes difficult to be precise as to which of these are the  cause of pain. Symptoms  of joint pain  vary from person to person and also depend  on the cause of the joint pain. However, despite  this variety, many people will  experience:

  • Stiffness, where movements  in the joint become more  limited because of pain
  • Swelling of the  joint
  • Loss of function  of the join
The severity of  your symptoms will  depend on the degree  of damage to your joint.  A joint that becomes warm to touch  suggests ongoing  severe inflammation or  infection – conditions which constitute  a medical emergency.

Causes of joint pain

Pain in one  or more joints can result from a variety  of relatively common health conditions. Whilst usually  associated with increasing age, joint pain is not  exclusive to the elderly.  For instance, a sporting injury  such as an ankle sprain can result in severe joint pain  with swelling in the joint, limitation of movement and the other symptoms described above.

The most  common causes of joint pain  seen in the general  public are;

Injuries

This is probably the  most common cause of joint pain and will  have been experienced by the majority of people  at some point in their lives.

Injuries to  joints can result from sports  injuries, falls, other simple  accidents such as ‘going over your ankle’  and other similar situations where forces exerted  on a joint exceed their  design capabilities.

The joint structures  most commonly damaged in these injuries  are the ligaments holding the bones  together. If the injury is  severe, more than one ligament may  be damaged, ruptured or severely torn.  Muscles and associated tendons surrounding the joints  may also become injured and inflamed. 

The most common of such  joint injuries is the sprained  ankle followed by damage  to the ligaments of the knee – the classic  footballer’s injury.

Osteoarthritis 

Osteoarthritis, sometimes  also referred to as ‘wear and tear’ arthritis, is probably  the second most common cause of joint  pain. The problem  is seen with  older people  and usually starts with one  or sometimes two joints.

The large, weight bearing  joints such as the hips or the  knees are typically the first ones to become painful.  Osteoarthritis of the  bones in the lumbar spine can give rise  to lumbago (low back pain) and similar  wear and tear of the spinal bones of the neck can, on occasion,  give rise to neck pain.

Rheumatoid arthritis

In contrast  to osteoarthritis, rheumatoid arthritis is  a condition which can give rise  to joint pain from a relatively  young age. It is a condition doctors  describe as an autoimmune illness – where  the body’s immune system,  usually designed to attack invading  organisms such as bacteria and  viruses, attacks its own cells.

Typically, rheumatoid  arthritis causes pain in a number  of joints simultaneously. Unlike osteoarthritis,  the smaller joints, such as those in the hands and feet,  tend to be involved first.

The symptoms  of rheumatoid arthritis can come  and go. ‘Flare ups’ of pain, are  interspersed with  quieter pain-free periods. It is a serious medical condition which should,  in the first instance,  be managed by a doctor or hospital specialist

Gout (High Uric Acid)

Gout is metabolic disorder  where uric acid crystals precipitate out of the blood and settle in joints and other tissues. Uric acid crystals  inside joints will set off and inflammatory reaction, giving rise to swelling and pain.

The joint of the big toes are classically  the first ones to be affected. Other joints such as the knees, elbows and finger can also be involved.

"It's shocking to  me how many women skip conditioner when showering, which is  the worst thing  you can do for your hair — especially when you're trying to grow it  long — and it's actually shampoo that you should be skipping  as much as possible,"  Townsend says. Here's why: The purpose of shampoo is to wash  away dirt and product buildup, but it can also take essential  natural oils that  keep strands soft and healthy along with it. When you really do need  shampoo, be gentler on your hair by only lathering up at your scalp and then simply  letting the suds slide down strands, hitting the rest of the hair  as the water rinses it away.


4. Use cool water if you shampoo

"If your hair is wet, that means you have to condition it," Townsend says. "Over time, from coloring  and heat styling, strands start to get thinner at the bottom." Conditioner  helps replace the lipids and proteins inside the hair shaft, "as well as  seal the cuticle to help prevent more damage from happening so that you  can get your hair to grow longer and look healthier," he says.


5. Massage your hair with smooth touch

Because hair  that touches your shoulders or beyond can be several years old and most likely needs more TLC than normal  conditioner. Townsend suggests using moisture-based masks and oils  weekly. "I make a natural oil treatment and give it to all of my clients to use  pre-shampoo," Townsend says. (His easy  recipe: 1 cup of unrefined coconut oil mixed with 1 tablespoon each of almond, macadamia and jojoba oil.) Because  oils can leave a residue on hair,  he says to apply it to damp hair, leave it on for 10 minutes, then shampoo and condition like normal. "These  oils are able to fill strands up with  fatty acids and then, when rinsed out, using regular shampoo helps seal them inside hair," Townsend says,  adding that Kate Bosworth, Diane Kruger, and Ashley and Mary Kate Olsen are fans. What is AIDS?

AIDS is a  medical condition. A person is diagnosed with AIDS when  their immune system is too weak to fight off infections.

Since AIDS was first identified in the early 1980s, an unprecedented  number of people have been affected by the global  AIDS epidemic. Today, there are an estimated  35.3 million people living with HIV and AIDS worldwide. 1

What causes AIDS?

Acquired Immune  Deficiency Syndrome, shortened AIDS, is caused by HIV. Some people may refer to AIDS as advanced  HIV infection.

HIV is a virus that  gradually attacks immune system cells. As HIV progressively damages these cells, the body becomes more  vulnerable to infections, which it will have difficulty in fighting off. It is at the point of very advanced HIV infection  that a person is said to have AIDS. If left untreated, it can take around ten years before HIV has damaged  the immune system enough for AIDS to develop.

Symptom of AIDS?

 A person is diagnosed with  AIDS when they have developed an AIDS related condition or symptom, called an opportunistic  infection, or an AIDS related cancer. The infections are called ‘opportunistic’ because they take  advantage of the opportunity offered by a weakened immune system.

It is possible for  someone to be diagnosed with AIDS even if they have not developed an opportunistic infection. AIDS  can be diagnosed when the number of immune system cells (CD4 cells) in the blood of an HIV positive person  drops below a certain level.

Curing AIDS?

Worryingly, many  people think there is a 'cure' for HIV, the virus that causes AIDS - which perhaps makes them take risks that  they otherwise wouldn't. However, there is still no cure for HIV. The only way to ensure that you avoid AIDS  is to be aware of how HIV is transmitted and how to prevent HIV infection.

However, people can  take antiretroviral treatment, which suppresses the HIV virus. This can prevent them from reaching a point  where they are diagnosed with AIDS. Also, in some cases, it is possible for a person who has an AIDS  diagnosis to regain a strong immune system with antiretroviral therapy. However, once somebody is diagnosed  with AIDS, officially they keep this diagnosis for life even though in  reality an opportunistic infection may  be cured or the number of immune system cells (CD4 cells) in their blood may increase to recommended  levels.

Since the  first cases of AIDS  were identified in 1981, more than 30 million people have died from AIDS. An estimated  1.7 million people died as a result of AIDS in 2011 alone.

Although there  is no cure for AIDS, HIV infection can be prevented, and those living with HIV can take antiretroviral  drugs to prevent or delay the onset of AIDS. However, in many countries across  the world access to  prevention and treatment services is limited. Global leaders have pledged to work towards universal  access to HIV prevention and care, so that millions of deaths can be averted.

AIDS treatment

Antiretroviral treatment can significantly prolong the lives of people living with HIV.  Modern combination therapy is highly effective and  someone with HIV who is taking treatment  could live for the rest of their life without  developing AIDS.

An AIDS  diagnosis does not necessarily equate to a death sentence. Many people can still benefit from starting antiretroviral therapy even once they  have developed an AIDS defining illness. Better treatment and prevention for opportunistic infections have also  helped to improve the quality and length of life for those diagnosed with  AIDS.

Treating some  opportunistic infections is easier than others. Infections such as herpes zoster and candidiasis of the mouth, throat  or vagina, can be managed effectively in most environments. On the other hand, more complex infections  such as toxoplasmosis, need advanced medical equipment and infrastructure, which are lacking in many  resource-poor areas.

It is also important  that treatment is provided for AIDS related pain, which is experienced by almost all people in the very  advanced stages of HIV infection.

Research AIDS now 

Even though a ntiretroviral treatment can prevent the onset of AIDS in a person living with HIV, many people are still diagnosed  with AIDS today. There are four main reasons for this:

In many resource-poor  countries antiretroviral treatment is not widely available. Even in wealthier countries, such as America, many  individuals are not covered by health insurance and cannot afford treatment.
Some people who became  infected with HIV in the early years of the epidemic before combination therapy was available, have  subsequently developed drug resistance and therefore have limited treatment options.
Many people are never  tested for HIV and only become aware they are infected with the virus once they have developed an AIDS  related illness. These people are at a higher risk of mortality, as they tend to respond less well to treatment at this stage.
Sometimes people taking  treatment are unable to adhere to, or tolerate the side effects of drugs.

In the later stages  of AIDS, a person will need palliative care and emotional support. In many parts of the world, friends, family  and AIDS organisations provide home based care. This is particularly the case in countries with high HIV  prevalence and overstretched healthcare systems.

End of life care becomes  necessary when a person has reached the very final stages of AIDS. At this stage, preparing for death and  open discussion about whether a person is going to die often helps in addressing concerns and ensuring  final wishes are followed.

 The Global AIDS Epidemic

Around 2.5 million  people became infected  with HIV in 2011. Sub-Saharan Africa has been hardest hit by the epidemic; in 2011 over two-thirds  of AIDS deaths were in this region.

The epidemic  has had a devastating impact on societies, economies and infrastructures. In countries most severely affected, life expectancy has been reduced by as much as 20  years. Young adults in their productive years are the most at-risk population, so many countries  have faced a slow-down in economic growth and an increase in household poverty. HIV and AIDS in  Asia causes a greater loss of productivity than any other disease. An adult's most productive years are also  their most reproductive and so many of the age group who have died from AIDS have left children  behind. In sub-Saharan Africa the AIDS epidemic has orphaned  nearly 15 million children.

In recent years, the  response to the epidemic has been intensified; from 2002-2008 spending on HIV and AIDS in low- and  middle-income countries increased 6-fold. Since 2008, spending has not increased so substantially, but it is  still significantly higher than it was before 2002. The number  of people on antiretroviral treatment has increased,  the annual number of AIDS deaths has declined, and the  global percentage of people infected with HIV  has stabilised.

However, recent achievements  should not lead to complacent attitudes. In all parts of the world, people living with HIV still face  AIDS related stigma and discrimination, and many  people still cannot access sufficient HIV treatment  and care. In Eastern Europe infection rates are rising,  and in Western Europe and America there are still  tens of thousands of new infections each year - indicating that HIV prevention is just as important now as it  ever has been. Prevention efforts that have proved to  be effective need to be scaled-up and treatment  targets reached. Commitments from national governments right down to the community level need to  be intensified and subsequently met, so that one day the world might see an end to the global AIDS  epidemic.

Baby Angelito has a rare disorder called the  Patau syndrome which happens once in every 10,000 to 15,000 births.

Paediatrician Jose Castillo said that the baby is  stable, breathing fine, and taking down the food being given to him.

The hospital’s neonatal care boss  Carlos Arrestegui Ramos said: “He doesn’t have difficulty breathing now, but he will need plastic surgery.”

The specific techniques applied  during the procedure were described at a press-conference in Lyon by the researchers from the startup, Kallistem, and French government lab CNRS.

The first announcement of  the discovery without any details was made in May.

So far, the group of scientists  has managed to create sperm from immature cells, also known as spermatogonial cells, which  can be found in all males, including prepubescent boys.

The method could be  soon used to help sterile men whose bodies do not produce sperm properly.

The research was initially triggered by alarming indications that due  to environmental factors, males’ ability to conceive has significantly declined over the years, Philippe Durand, the chief Kallistem researcher said, according to NewsOK.



First thing they tried to create in the lab was the interior of a testicle, since it is “at the heart of the problem,” he explained.

Using a mixture of various fluids  including those found in mushrooms, or in crustacean shells, they simulated the conditions in the human body and  came up with a so-called bioreactor.

The experiment was first carried out with rat cells, then young monkey cells and finally human cells.

The next, and apparently most challenging, step  was to turn cells into sperm – a process that lasts more than 70 days in a male body.

“They [the cells] took the entire path they would have taken in the testicle in our in-vitro system,” said Durand.

The technique is also a chance for children  who are about to start dangerous treatment such as chemo to preserve their fertility, CNRS researcher  Marie-Helen Perrard added.

Immature cells could be frozen, to be later turned i nto mature sperm and used when the time comes when the children would like to have their own kids.

Clinical trials are planned for 2017. By that  time the team hopes to get permission to use artificial sperm to fertilize a human egg and settle all ethical  issues that may arise.

“We must make sure it’s safe, ethical,  secure and acceptable for the patients and regulatory agencies,” says Perrard.
